Complete preventive screenings this year for entries into the 25 Days of Giveaways in December - twenty-five $100 Amazon gift cards!
Preventive care is important because helps you keep an eye on your body’s numbers (blood pressure, cholesterol, body mass index, and blood sugar) to help you identify symptoms that could develop into more serious illnesses. Basic preventive care includes completing your annual wellness exam. At that appointment, your physician will recommend other preventive screenings that are appropriate for you based on age, gender and as needed.

The schedule of screenings (as noted above with an asterisk*) should be adjusted based on your family’s medical history and your individual risk for ailments based on age or lifestyle choices (smoking, drinking, weight, or activity levels).
25 DAYS OF GIVEAWAYS
When you complete any of your recommended screenings by October 31, 2025, you’ll be entered into the 25 Days of Giveaways – one entry for each completed visit.
Each completed preventive screening (examples: annual wellness visit, mammogram, colonoscopy) will enter you into a daily drawing for a $100 Amazon gift card. Each entry is another chance to win – so schedule your wellness visit and screenings to load up on chances to win! There aren't any forms to fill out. When you complete your screenings, you'll show up on the monthly report from Anthem. And, don't worry - no personal health information is shared, just that you completed a preventive screening.
